Before you share anything with your clients, please take a moment to tailor each template to your practice, and have it reviewed by your licensing board, malpractice carrier, and/or a licensed attorney in your state. This small step protects both you and the people you serve.
Built to be made yours
We've done the structural work, and you bring the personal touch. Red-bracketed placeholders mark every field to fill in, and teal clinician notes explain the purpose of each section and the clinical reasoning behind it. Read the notes, make your choices, and delete them before sending to a client.
HIPAA is the floor. Your state sets the ceiling
Many states layer on stricter rules around mental health records, minor consent, telehealth licensing, and confidentiality. The teal notes flag where these variations come into play and prompt you to fill in what applies locally. They are a head start, not a substitute for knowing your state's specific requirements.
Review before launch, refresh annually
Each template ends with a prompt to have your finished version reviewed by your licensing board, malpractice carrier, and/or a licensed attorney before you distribute it. After that initial review, mental health law, telehealth regulation, and HIPAA guidance continue to shift, so we recommend revisiting your finalized versions once a year so they keep pace with current requirements.
Written for clinicians practicing in scope
This system supports licensed mental health clinicians working within their scope of competence. Some sections, including trauma screening, substance use, and spirituality and worldview, reflect best practice at intake. Include them where they align with your training and licensure; when in doubt, remove the section or consult a supervisor or colleague.
