I've done the structural work so you don't have to start from scratch. Red-bracketed placeholders mark what needs your information. Teal clinician notes explain the purpose of each section and the reasoning behind it. Read them, make your choices, delete them. What's left will feel like it came from you, because it did.

Federal law is just the beginning. Many states have stricter rules around mental health records, minor consent, telehealth, and confidentiality, and those differences matter. The teal notes flag where state-level variations tend to come up and prompt you to fill in what applies where you practice. A solid head start, not a substitute for knowing your own state's rules.
Review before launch, revisit every year
Each template closes with a prompt to have your finished version reviewed by your licensing board, malpractice carrier, and a licensed attorney before it goes to clients. That first review matters, and so does the annual one. Mental health law, telehealth regulation, and HIPAA guidance keep shifting, and your documents should keep up.
Know your scope, trust your training
These templates are built for licensed mental health clinicians working within their scope of competence. Some sections, including trauma screening, substance use history, and spirituality and worldview, reflect best practice at intake. Use what aligns with your training and licensure. When in doubt, pull the section or run it by a supervisor before it goes to a client.
